Carroll County Coalition for Public Health

Carroll County Coalition for Public Health (C3PH) is a non-profit organization based in Ossipee, New Hampshire. With four employees, C3PH works to improve the health and well-being of the communities within Carroll County. Since its establishment, the organization has held a contract with the New Hampshire Department of Health and Human Services (NH DHHS), managed by Granite United Way, to oversee a range of public health services and functions. C3PH provides a variety of essential public health services, including those mandated by the state. These services are delivered proactively, in a coordinated manner, and comprehensibly to benefit all residents within their designated region. The organization serves nineteen distinct communities across Carroll County: Albany, Bartlett, Brookfield, Chatham, Conway, Eaton, Effingham, Freedom, Hart's Location, Hale's Location, Jackson, Madison, Moultonborough, Ossipee, Sandwich, Tamworth, Tuftonboro, Wakefield, and Wolfeboro. As one of thirteen Regional Public Health Networks across New Hampshire, C3PH plays a vital role in ensuring the health and well-being of its communities.
