The Coordinating Center

The Coordinating Center is a non-profit organization located in Severna Park, Maryland. Founded in 1983, the organization has grown to employ 184 individuals. The Coordinating Center partners with people of all ages and abilities, as well as their support systems, to help them achieve their goals for independence, health, and meaningful community life. They create community-based, culturally competent care models for individuals with complex healthcare needs, disabilities, and psychosocial needs. The Coordinating Center's programs address a range of areas including transitions of care, Medicaid rebalancing initiatives, aging in place, reducing hospital readmissions, and addressing homelessness. The Coordinating Center has a history of translating national movements into localized, impactful programs. They currently provide care coordination services to over 10,000 individuals across Maryland and collaborate with various public and healthcare agencies to improve patient outcomes.
