Snowline Health

Snowline Health is a nonprofit organization based in Diamond Springs, California. Established in 1979, Snowline employs 105 individuals and provides comfort, care, and support to individuals facing chronic, serious illness and end-of-life transitions. Snowline Health offers end-of-life care services to patients and families through an interdisciplinary team consisting of physicians, nurse practitioners, registered nurses, licensed vocational nurses, medical social workers, spiritual care providers, hospice aides, and volunteers. Their dedicated staff specializes in hospice care, with many holding advanced certifications from the Hospice and Palliative Credentialing Center and the Hospice and Palliative Nurses Association. Snowline Health is a proud Employer Partner with the Hospice and Palliative Nurses Association and encourages staff professional development through certification. Hospice benefits are typically covered by Medicare, Medi-Cal, and most health insurance plans. For those without insurance or requiring palliative care, Snowline provides services through funds generated from thrift stores, a recycle and repurpose processing center, individual donations, fundraisers, and the United Way. Notably, Snowline Health never bills patients or their families for services. Snowline Health serves the western slope of El Dorado, Placer, and Sacramento Counties, including Sacramento, Folsom, Fair Oaks, Elk Grove, Roseville, Granite Bay, Fair Oaks, Carmichael, Lincoln, Rancho Cordova, and Rancho Murieta.
